The closing date for receipt of completed entry forms is 5pm on Wednesday 9 March 2005. Entries may be made in more than one category (for multiple entries please submit each entry on a separate form) and must address the criteria established for each category, against which the judges will make their decision.

Return a hard copy by post and ensure that you provide 5 copies of your submission and the same number of copies of any supporting evidence you wish to include. These should be attached to your submission.
